Ear Infections in Children: Symptoms, Treatment, and Prevention
How to recognize ear infections in kids, when antibiotics are needed, and prevention tips from the board-certified pediatricians at American Pediatrics.
Ear infections are one of the most common reasons for pediatric visits — by age 3, about 75% of children will have had at least one. Understanding the signs and treatment options helps you respond confidently.
Types of Ear Infections
Acute otitis media (middle ear infection) is the most common type in children. Fluid builds up behind the eardrum, often following a cold, causing pain and sometimes fever. Otitis externa (swimmer's ear) affects the outer ear canal and is common after swimming.
Symptoms by Age
Infants: Fussiness, pulling at ears, difficulty sleeping, fever, decreased appetite. Toddlers: Ear pain, trouble hearing, fluid draining from the ear, balance problems. Older children: Can usually tell you their ear hurts, may describe a feeling of fullness or muffled hearing.
Do They Always Need Antibiotics?
Not always. The AAP recommends a "watchful waiting" approach for children over 2 with mild symptoms in one ear. Many ear infections resolve on their own within 48–72 hours. However, antibiotics are recommended for children under 6 months, children with severe symptoms, bilateral infections, or if symptoms don't improve within 48–72 hours.
Pain Management at Home
Use acetaminophen or ibuprofen for pain relief. A warm (not hot) compress held against the ear can provide comfort. Keep your child's head elevated. Avoid putting anything inside the ear canal unless directed by your pediatrician.
Prevention Tips
Breastfeeding for at least 6 months reduces ear infection risk. Keep children away from secondhand smoke. Stay current on vaccines (flu and pneumococcal vaccines help prevent ear infections). Don't prop bottles during feeding.
If your child has recurrent ear infections (3 in 6 months or 4 in a year), talk to your American Pediatrics provider about whether ear tubes might be beneficial.
